Use this form if you’re having technical difficulties with the website such as problems purchasing a class or to report missing or incorrect information, etc.

BEFORE SENDING A REPORT, please try the following steps first if you had a product purchase or other form technical issue (for missing or incorrect website information, please skip ahead and describe the issue in the large field below):

  • Restart: Browsers and operating systems crash occasionally. Try restarting your browser or computer/phone/tablet, then try the webpage again.
  • Browsers: Try a different browser such as switching from Safari to Firefox or Chrome or try another device.
  • Connection: Ensure you have a solid internet connection. If on WiFi, try switching to a cellular network temporarily or vice versa. Proxy networks and firewalls can be problematic too. Ask your network admin for help with these.
If you have a current (or past) AFSR Membership, please use that account email if known.
Please include a phone number and the best time to call.
Please be as detailed as possible about the problem. Here are some prompts in order of priority to get started (there's no need to answer all, rather, pick ones that seem most important):
  • • The page Title and URL (or web address) where the problem occurred
  • • Any error codes or other on-screen text that appeared about the problem
  • • If the problem occurred during a class purchase, at what stage did the problem happen (such as selecting an option or logging in or choosing a payment method, etc.)
  • • The kind of payment method used
  • • What you expected to happen
  • • Kind of browser and its version number (Safari 17.5, Chrome 47.x, Firefox 127.x, etc.)
  • • Your device type (laptop/phone/tablet/etc.), model, and manufacturer (iPad Mini 6, MS Surface, etc.)
  • • Computer OS version and its version (Mac OS 14.5, iOS 17, Windows 11, etc.)
  • • One or more screenshots attached to the Screenshot(s) file upload field below is very helpful
